Porter dentist offers free oral cancer screenings for firefighters

As a thank you to local first responders, Porter Family Dentistry is offering free oral cancer screenings to firefighters in Montgomery County for the next several weeks.

The screenings will be held on Fridays when the office is usually closed so that firefighters don’t have to wait.

In 2016, the National Institute for Occupational Safety and Health published a multi-year study of cancer rates in firefighters, and the findings showed that firefighters had a higher number of cancer diagnoses and cancer-related deaths than the general U.S. population. Among the cancers found in the sample of nearly 30,000 firefighters, those most often found were digestive, oral, respiratory, and urinary cancers.

In recent months, firefighters across the country have been traveling to areas, like California, that their help is needed. Dr. Mustafa Yamani of Porter Family Dentistry went to school in California and has fond memories of the nature and beauty of the state.

The generous act of the firefighters inspired him to give back, and since oral cancer screenings are a service his office already offers he decided to give them to firefighters for free.

While this is the first year that the dental office has offered free screenings, Yamani and his wife Sabrina, who is the office manager, plan on making it an annual thing. The trials of 2020 also helped them decide to give back.

“Things seem to be going from bad to worse, to even worse, and it just doesn’t seem to be stopping for our first responders,” Sabrina said. “They’re just being hit with things one after the other.”

Every day we go and put our lives on the line for the community, and for them to give back and worry about our safety is something we’re not used to but it’s much appreciated.”

Evinacumab Offers ‘Remarkable’ Lipid Lowering in Severe HoFH

Evinacumab (Regeneron) appears to have a dramatic effect on low-density lipoprotein (LDL) cholesterol levels in patients with homozygous familial hypercholesterolemia (HoFH) with little or no LDL receptor activity, suggests a post hoc analysis of phase 3 trial data.

The results, which one expert described as a “game changer” for these patients, were presented at the European Atherosclerosis Society 2020 Virtual Congress on October 5, held online this year because of the COVID-19 pandemic.

The phase 3 ELIPSE trial showed that evinacumab, a human monoclonal antibody inhibitor of angiopoietin-like 3 (ANGPTL3), given intravenously every 4 weeks reduced LDL cholesterol levels in HoFH patients by an average of 47%.

As reported by | Medscape Cardiology, the treatment, which was generally well tolerated, was also effective in the approximately one third of patients with minimal residual LDL receptor activity.

Now, Frederick Raal, MD, PhD, the University of the Witwatersrand, Johannesburg, South Africa, presented a post hoc analysis of 10 patients with less than 2% functional LDL receptor activity. These patients experienced an even greater reduction of 72% over placebo.

Raal said that, taken together, the results are “remarkable in these very, very difficult to treat patients,” who have “a very unmet need.” He noted that the drug is “generally well tolerated.”

“Evinacumab may provide an effective treatment option for patients with HoFH who are unable to reach target LDL cholesterol levels despite multiple lipid lowering therapies, with or without apheresis,” Raal concluded.

Anne C. Goldberg, MD, a lipidologist and professor of medicine at Washington University in St. Louis, St. Louis, Missouri, told | Medscape Cardiology that HoFH patients with little or no LDL receptor activity, who number in the hundreds around the world, are a “very difficult group to treat because things don’t work.”

She pointed out that over 90% of patients in the trial were aged in their 30s and that the majority of them already had heart disease.

“So if you have something where you could start treating them relatively young and safely, theoretically, you could try to prevent some of these terrible cardiovascular outcomes in this group,” she said.

Goldberg highlighted that a drug such as evinacumab that can lower LDL levels in patients, often “for the first time ever, is quite a game changer.” She said that this was also seen with anti-proprotein convertase subtilisin/kexin type 9 (PCSK9) therapies among patients with HoFH.

She also noted that the safety profile of evinacumab is encouraging.

“There are always going to be people with problems with anything, but there are not a lot of side effects,” she said, underlining that this is not seen “that often.”

Summarizing, Goldberg said that evinacumab “just seems to work, I think pretty nicely,” although she added that it “would be nice if we had a better idea about the mechanism of action, because it’s still a bit unclear how it works.”

HCA Houston Healthcare North Cypress offers preventative gene testing for breast cancer

When Dalyss Tomayer found out three of her relatives had had breast cancer, it was enough to qualify her for the genetic testing which showed she had a high risk of breast cancer herself.

Tomayer was able to discover the gene early due to getting tested at HCA Houston Healthcare North Cypress’ High Risk Breast Clinic, where they offer genetic testing for those who qualify to see if they may have the BRCA1 or BRCA2 gene which puts people at higher risk for breast cancer.

“I had known for years that two of my dad’s sisters had breast cancer, and it was in speaking with one of my aunts that I then found out also one of my cousins had breast cancer,” Tomayer said. “So, it was at that time that I realize that I did qualified for the genetic testing and I decided to go ahead and do it.”

Janet Pollard, the coordinator for the High Risk Breast Clinic, said the breast center began as a full program under HCA Houston North Cypress’ CEO Jim Brown, who she said was passionate about having this resource.

“Most of the time people think about genetic testing after you get cancer,” Pollard said. “It can drive the type of surgeries, it can change treatment regime, but basically it’s getting it done prior. It’s screening people when they come in for their annual mammograms, talking with primary care doctors, educating them on what makes patients meet criteria.”

Criteria established by the American Cancer Society for genetic testing includes people who were diagnosed with breast cancer at a younger age, those diagnosed with breast cancer a second time, people of Ashkenazi Jewish descent, people with a family history of breast cancer, ovarian cancer, pancreatic cancer or prostate cancer, and people with a known family history of BRCA mutation.

Before the pandemic, Pollard said the center was testing around 30 people a month, men and women, with about 10 percent of people testing coming back positive.

“BRCA1 and 2 genes are huge red flags,” Pollard said. “They’re the original genes and they’ve been testing those genes for two decades.”

The process Tomayer chose to go through after being tested was to have a double mastectomy to virtually remove all chance of breast cancer, as well as a hysterectomy to eliminate her chances of ovarian cancer.

“Even though you do a double mastectomy, there’s no way to get 100 percent of every breast cell in my body, so there is still a slight chance, but it is incredibly slight,” she said.

Tomayer said she would continue to have ultrasounds for what remaining breast tissue she has left.

“I’m so very thankful they have the test,” Tomayer said. “It saved my life.”

SEAL BEACH, CA — Need a flu shot? or a coronavirus test? Rite Aid now offers flu shots and has opened additional, no-charge COVID-19 drive-thru testing at pharmacies across the state.

This year isn’t like most in the past and health officials are encouraging people to get a flu shot.

“Getting a flu shot early is more important than ever this year to maintain a healthy immune system as the COVID-19 pandemic and flu season converge,” the company said.

Several new Rite Aid drive-thru testing sites were added in California in September:

Russian President Vladimir Putin is extending wishes of a speedy recovery to U.S. President Donald Trump and his wife, Melania, and expressing “sincere support in this difficult moment,” according to a statement released by the Kremlin on Friday.

Trump announced on Twitter early Friday that he and first lady Melania Trump had tested positive for the coronavirus.

The Kremlin says Putin sent Trump a telegram saying, “I hope that your inherent vitality, good spirits and optimism will help you cope with the dangerous virus.”


5 a.m.

U.S. Secretary of State Mike Pompeo says he and his wife have tested negative for the coronavirus after they were examined on their airplane 20 minutes prior to landing in Dubrovnik, Croatia, on Friday.

He said it was the fourth time in two weeks he has been tested.

President Donald Trump announced on Twitter early Friday that he and first lady Melania Trump had tested positive for the coronavirus.

Pompeo says the last time he was with Trump was on Sept. 15, at the White House, for the signing of normalization agreements among Israel, the United Arab Emirates and Bahrain.

The top U.S. diplomat says he is reconsidering upcoming travel to Florida on Saturday and Asia starting Sunday as a precaution.

He says, “We are praying for the president and the First Lady and we hope they have a speedy recovery.”

British Prime Minister Boris Johnson is wishing U.S. President Donald Trump a “speedy recovery” from COVID-19.

Johnson tweeted Friday morning: “My best wishes to President Trump and the First Lady. Hope they both have a speedy recovery from coronavirus.”

Johnson was hospitalized for a week in April after he contracted COVID-19, the disease caused by the new coronavirus. After he was released, the prime minister thanked doctors and nurses at St. Thomas’s Hospital for saving his life. Johnson was treated in the hospital’s intensive care unit, where he received oxygen but was not put on a ventilator.

Russia currently has the fourth largest coronavirus caseload in the world with over 1.19 million confirmed cases and more than 21,000 deaths. A major study in India offers insights into on how the spread of the virus differs by age and gender.

An ambitious new study of nearly 85,000 coronavirus cases in India and nearly 600,000 of their contacts, published Wednesday in the journal Science, offers important insights not just for India, but for other low- and middle-income countries.

India now has more than six million cases, second only to the United States.

Among the findings of the study: The median hospital stay before death from Covid-19, the illness caused by the coronavirus, was five days in India, compared with two weeks in the United States, possibly because of limited access to quality care. And the trend in increasing deaths with age seemed to drop off after age 65 — perhaps because Indians who live past that age tend to be relatively wealthy and have access to good health care.

The contact tracing study also found that children of all ages can become infected with the coronavirus and spread it to others — offering compelling evidence on one of the most divisive questions about the virus.

“The claims that children have no role in the infection process are certainly not correct,” said Dr. Joseph Lewnard, an epidemiologist at the University of California, Berkeley, who led the study. “There’s, granted, not an enormous number of kids in the contact tracing data, but those who are in it are certainly transmitting.”

And the report confirmed, as other studies have, that a small number of people are responsible for seeding a vast majority of new infections.

Though its overall total of cases is huge, the per capita number of cases reported daily in India — and in many other low-income countries, including in Africa — is lower than in Spain, France or even the United States. And its number of deaths has not yet topped 100,000 — which has surprised some scientists.

The study focused on two southern Indian states, Andhra Pradesh and Tamil Nadu, which together have a population of about 128 million, and represent two of the five Indian states with the most cases. They also have among the most sophisticated health care systems in the country.

Contact tracers reached more than three million contacts of the 435,539 cases in these two states, although this still did not represent the full set of contacts. The researchers analyzed data for the 575,071 contacts for whom test information was available.

The data revealed that the people infected first — known as index cases — were more likely to be male and older than their contacts. That may be because men are more likely to be out in situations where they might be infected, more likely to become symptomatic and get tested if they do become infected, or perhaps more likely to respond to contact tracers’ calls for information, said Dr. Lewnard. They also found that infected people tend to spread the virus to those of similar ages.
